David Phak, PMHNP- BC, helps adults and older adults across California regain stability, clarity, and confidence through individualized psychiatric medication management. Practicing at LifeStance Health, he supports patients throughout California who are navigating complex mental health conditions, offering careful evaluation, collaborative treatment planning, and ongoing support focused on long-term wellness and daily functioning.

David is especially committed to working with individuals who have been recently discharged from a psychiatric hospital. He understands that the transition back into daily life can feel overwhelming and requires close follow-up, clear communication, and thoughtful medication oversight. His approach emphasizes continuity of care, symptom stabilization, and practical strategies that help reduce the risk of relapse while rebuilding independence, structure, and confidence after hospitalization.

His clinical focus includes ADHD, bipolar disorder, and PTSD/trauma, with experience treating anxiety, depression, and other serious mental health conditions as part of comprehensive care. David provides medication management for adults and seniors and works with diverse populations, including first responders, military veterans, and members of the LGBTQ+ community. He brings a culturally responsive lens to care and also offers services in Khmer. While medication management is central to his practice, he supports a wide range of additional psychiatric concerns when clinically appropriate.
